Make AN EMERGE Account
In order to book any class or service at Emerge, you need to register with our scheduling system, MindBody. This is where you can register for classes, book appointments, and schedule other events and services.
This all you need to enjoy our classes and workshops.
However, for therapeutic services, additional forms will be required.
If you are receiving, acupuncture, massage therapy, or nutrition counseling, additional intakes and/or insurance billing documents will be e-mailed directly to you. You will need to sign into our Patient Portal [this is different than your MindBody account] to fill out the appropriate waivers.
Please check your SPAM folder if you do not receive the link to our online waivers after booking.
All required documents must be submitted prior to your appointment. If you have any issues accessing them at home, simply arrive 20 minutes prior to your scheduled appointment and a Studio Director will help you access them at the front desk.
MINORS (under 18 years old):
Parents or guardians must fill out the form with the minor’s information and sign the waiver on their behalf.
Please Note: If the parent/guardian has their own MindBody account, a separate e-mail must be used for the minor child. If you do not have a second e-mail address to use, simply write “child’s first & last name @emerge.com” as the e-mail. It doesn’t matter that it is an inactive address, we simply need a unique login for each client in our system.
